Senior Care Managers perform individualized person-centered assessments and then work with their client, their loved ones, and the community to find the resources and services available for each senior.
Whatever your need we are here to understand what is needed and what is available. Whether it’s a need for companionship, in home care services, in home repairs, increased security, transportation, assistance to find the best assisted living or nursing home for you, legal advice, preparation of wills and power of attorney, appointment coordination, medical appointment escorts, or a desire to be more active in the community, a Senior Care Manager will put you or your loved one in the center of the care plan to bring the services and programs to each senior. Always keeping in mind what already exists to meet the need in the community and what can be accomplished first through family and volunteers, community agencies, and then through paid private services.
We have trusted partners in the community to meet your needs. All our community partners offer at minimum a 5% discount to our clients, undergo criminal background checks, and complete an in-service training that highlights best practices when working with seniors. Plus, all service provides agree to allow Community Light Elder Network to help resolve any complaints through our complaint process.

Please note, you do not have to be a client or a member to review our trusted partners. All service partners are available to be utilized without going through Community Light Elder Network, however to receive the 5% discount you would need to be a member of your local Senior Center or Community Light Elder Network.
*We encourage all seniors to join their local senior coalition or senior center and to utilize the case managers available in the community. The case managers at the senior centers and coalitions are a valued resource for seniors and often the only outside case manager needed. If however you prefer a private senior care manager or need a more in depth care manager, we are here. Also, often the Aging and Disability Resource Center can help guide seniors to the resources they need and can be the first step when starting to research services.
